Mohan's Story: A Breath of Relief in the Digital Age

For Mohan Prasad, an auto mechanic from Singrauli district, a lost prescription meant a potentially dangerous delay in acquiring his asthma medication. But thanks to the Madhya Pradesh Health System Strengthening Project (MP-HSSP)’s digitalization initiative, his anxiety was quickly quelled. His medical history, meticulously recorded in the digital system, allowed his doctor to readily access his information and provide the necessary treatment. 

Mohan’s story exemplifies the transformative power of digital healthcare. By leveraging existing digital platforms, the MP-HSSP is ensuring comprehensive and continuous care for patients, creating a “future-ready” healthcare system. This digitization, rolled out in phases, promises a paradigm shift in healthcare delivery.

Subba Lakshmi: Empowering Change through Data

Subba Lakshmi, a junior secretary assistant at the Mathpurena Urban Primary Health Care Centre (M-UPHC), embodies another facet of digitalization’s impact. Armed with the CPHC NCD platform, a software developed in collaboration with the Ministry of Health and Family Welfare (MoHFW), Tata Trusts, and Dell Technologies, Subba empowers the center by effectively screening and recording data on non-communicable diseases (NCDs) like hypertension and diabetes. 

This user-friendly platform, accessible on tablets, mobiles, and web-based software, is designed to strengthen the Ayushman Bharat Comprehensive Primary Healthcare (CPHC) program. It aims to screen individuals over 30 years old and facilitate timely referrals to higher-level facilities for diagnosis, treatment, and management. 

Through this platform, the M-UPHC, despite facing staffing and infrastructure limitations, has been able to: 

  • Identify NCDs early: Early detection allows for prompt treatment, potentially saving lives. 
  • Regularly follow up with patients: This ensures adherence to treatment plans and monitors patients’ progress. 
  • Recommend home-based care: This caters to the needs of patients who may struggle to visit the center regularly. 
  • Strengthen training sessions: The data collected can inform targeted training programs for healthcare workers, improving overall service delivery.

From empowering patients like Mohan to streamlining data management and enabling data-driven decision-making like Subba, the shift towards digitalization is paving the way for a more efficient, accessible, and patient-centered healthcare system.
